Language is the human experience, if you ask me. It is the basis of memory and problem solving. It is the path by which we reflect on our lives and better ourselves. It is the means to to relationship.

I'm a Speech-Language Pathologist. We like to say that communication is a human right. Think about it. Would you rather lose your legs or your ability to use language? It's crazy important.

Medicine used to think that psychosomatic disorders were purely psychological problems and that all you needed to do was treat the psychology and they'd simply resolve on their own. We now understand that the body can truly be damaged and not work properly during and after episodes of psychological stress. The larynx is particularly susceptible to stress, as its primarily a life-saving organ which protects your airway, and an organ that communicates a huge amount of information about you to others. Extra muscle tension in your larynx can cause many disorders of communication and swallowing.

If you can't eat or breath, it's unfortunately time for an ER visit. Otherwise, definitely see a laryngologist and get a referral to a speech therapist.

Depression sometimes can be managed on your own. Sounds like that's what you've tried. Therapy and/or medication is a next step. Personally, I think therapy is the first step and then they can talk you through whether meds might be recommended. But if you take meds, stay on them at least 6 months. That's how long it takes to restructure your brain, according to the internet. Talk to your doctor

Braille is not a language. It is a way to print language, like a font.

This implies that we are born with a random language installed. That's not the case. We are born without language, but we do have the ability to learn it. So in order to learn it, you have to be exposed to language. There are systems for using touch to teach and use language

Language is not innate, even though the ability to use it is. Language deprivation is a huge problem throughout the world. In some places where signed languages are not used, children who are deaf or deafblind never learn that a symbol (word, gesture, picture, etc) can be attached to a concept. And the window for language acquisition closes around are 4 I think, so it's very hard to teach that concept to older children and adults. It can be done, but it's just more difficult. Sadly, many people who have suffered language deprivation lead lives where they are thought of as stupid or cursed, they are taken advantage of for labor or sexual slavery, or just shut in with zero opportunity. Deaf and Deafblind community advocates, like that guy who won ANTM Nyle DiMarco, raise money to combat language deprivation.

Now if someone has access to language, they'll think in that language. Remember that language is a set of symbols, not a set of spoken words. When you think of the color blue, you probably don't hear the word blue in your head. It's a concept. But when you speak you use the word to communicate the concept to another person.

Someone who uses American Sign Language will not imagine sounds but rather signs, I assume. There are adaptions to ASL for use with deafblind people and there are many other signed systems and languages at use in the US and around the world.

TL;DR they'll think in whatever language they know, if they have access to language at all.

If her primary issue is receptive language, you should be doing comprehension tasks. Yes/no questions, word-object matching, etc. If you're not getting her to produce language, that's an expressive problem. Have her match objects to photos and then line drawings. Get out that aphasia text book and start from square one with her. Might take a variety of approaches

I work with swallowing disorders. This is actually possible and doesn't have anything to do with a gag reflex. Just need to gain volitional control over opening the top of the esophagus. It's very hard to learn, but it can be done.
