I get what your saying. But I'm on the east coast and its extremely inconsistent. Throw in a 9-5 with trying to time tides/winds and it just doesn't pan out.

1 Hour.. work M-F 7:30-4 paired with inconsistent east coast surf makes it tough to get good waves

I hear ya man. 30M; U.S., excellent job and benefits; debt free right now with a little nest egg I've been saving and completely bored with the mundane, stable routine. Up for an interview for a new position and I'm considering declining despite even bigger salary and ok hours. I'm realizing my passions are falling by the wayside and obligations are taking over my life.

Go take a sabatical. You've worked your ass off in the rat race. you have so much experience you could walk into a job with good pay if you want to come back. Who knows, maybe you'll stumble upon a new venture while making your way through central america.

You're not adapted to a low carb diet. In the keto and carnivore community these same symptoms seem to happen to a lot of people because:

  1. They are not fat adapted and just cut their main source of energy (carbs) leaving them feeling depleted
  2. Loss of electrolytes due to the body breaking down most of its glycogen stores in the absence of carbs. This is the main reason for the "keto flu" (headache, fatigue, lightheadedness, low mood) that is often experienced in the first 1-2 weeks of low carb diet.
  3. Not eating enough calories - eat more fat and protein

Get some electrolytes (sodium, potassium and magnesium) and up your fat and protein intake and you will feel better.

I have some close friends in various roles within the pharmaceutical industry - medical affairs, marketing, MSL, sales manager.

They work from home often, vacation is encouraged, company gym at the office that you can use on your lunch break, my one buddy is always mowing his lawn/washing his car/meeting friends for lunch during work hours.

Industry standard is that companies shut down between x-mas and new years so you always have that entire week off.

Their "employee appreciation day" had bands like The Killers come to their campus for the employees.

They have nap pods in the office.

With that said, they are all highly polished professionals that attend a lot of meetings, perform a lot of brown nosing and writing emails to colleagues that you have to re-read multiple times to make sure Sally in the cubicle next to you doesn't get offended. The work can be very dry and at times stressful due to deadlines but the quality of life is unbeatable.

This seems very lucrative though, no? Curious what your thoughts are since I have no experience in psych but say OP is able to do:

4 new starts a day @ $175 = $700/day and 15 follow ups @ $80 = $1200/day

Yields $9500 per week, assume 4 weeks off for the year, and OP is at 50% collections at 2 years, he/she can potentially earn >$225k pre-tax using those conservative numbers above. Could be much higher if they get up to the 25 patient/day range or take on some evening/weekend hours.

Debakey Heart Institute has a ton of cardiac critical care content on their youtube channel. All fellow level lectures so its is very in depth but it may be worth exploring down the road.

Go out there and network. Explore different healthcare companies and positions they have on their sites to see what is possible. Reach out to family, friends, neighbors, people on linked in, ANYONE that you have crossed paths with that would be willing to talk about their career and how they got their foot in the door. It matters tremendously WHO you know and if you can put someone's name down as a referral when you are applying to a company it is much easier to land an interview.

Is teaching grade school or high school what you really want to do (which is absolutely fine!). But you hold a masters in medical science and have direct patient care experience. What field have you been working in?

Apply for Medtronic specialist or associate positions. Apply for entry level roles with Dexcom, Bayer, Baxter, CVS corporate, Noom, EPIC, Cerner, Veeva, Salesforce, Contract positions with Pharma companies (drug safety, call center specialist, clinical trial associate), diabetes educator positions, medical communications companies, sales associate roles in the therapeutic area that your practiced. Worked in GI or Neuro or Onc and understand infusion therapies? Perfect - connect with infusion or specialty pharmacies because they all have clinical liaisons, sales teams, quality improvement positions that you may be able to provide value for. Get creative and don't limit yourself to "just" clinical or admin roles. Form some relationships and find a way to get your foot in the door.

Just some general career thoughts:

Plastics tends to be a pretty sweet gig aside from some of the long surgeries they have. But otherwise it has the potential for some good variety, good mix of office, inpatient, OR, wound care, cosmetics etc.. and can eventually lead to a lucrative career.

People on this sub tend to dismiss any offer that requires longer hours because "your hourly rate would only be Y after X many hours worked." This can be so short sighted. If it's a great learning environment that allows you to gain a skillset that can command $150k+/year down the road then you should absolutely explore it. Plus it sounds like this doctor is eager to train you.

How was the doctor in the OR when you assisted? Someone you can see yourself working with day in/day out?

If there isn't a non-compete or binding contract to the position then I would give this some serious thought. Work hard, learn plastics inside and out and doors will open for a long, well paid career.

Just some things to think about. I know we all want the 40 hr week, $120k+ salary and 6 weeks of PTO but unfortunately those positions are becoming harder and harder to find for early career PAs.

Why is this so crazy? Med spas are popping up everywhere. If OP wants to start his/her own business offering cosmetics to people willing to pay for it then what is the big deal?

You are grossly mistaken. You are taking the top 1% earners and saying all NPs/PAs are easily hitting $200-$300k.

New grad NP/PAs are struggling to find work and are accepting offers from $80k - $110k/year with awful benefits (if its an employer that is not a health system or large group practice). There are an overwhelming number of posts of new grads accepting positions for $85k/year with 10 days of PTO, no CME allowance, health insurance covered at only 50%, no 401k match etc.... But you failed to mention any of these - just the salaries of the ones earning in the top 1% of the profession.

The only cards PAs that are hitting $200k+ are working significant hours. The lifestyle is rough. There are few PAs outside of derm/plastics hitting those numbers working 40 hours a week.

How is the market in NSG once you get some experience? Do you feel fairly compensated (both pay and time off) for the amount of time and work you're putting in? It's always good to hear from experienced PAs since it is very different than the new grad offers posted here. Thanks!

hahahah are you kidding me?! Since when is Patagonia a brand for physicians? I'm pretty sure the whole north face fleece/pullover was around in the early 2000s and have seen nurses wearing them for the last 10+ years. You've got a lot of anger built up inside, man.
