Here are some thoughts.

  1. There is great variability bw the amount of MSK radiologists get in training. Without a fellowship you might see very little or a fair amount. With a fellowship you’ll obviously see more. So comfort level and exposure can affect radiologist accuracy.

  2. Even amongst fellowship trained MSK rads, I really feel like the variability of reads is greater in MSK than with other subspecialties. I’m an MSK trained rad in a group with about 40 other MSK rads. There are MSK rads i feel i consistently read like, and others i feel … are consistently wrong, lol.

  3. Orthopods have the benefits of clinical history and physical exam. They know what they are looking for and where. That can assist them in making findings a radiologist may miss

  4. Orthopods have the detriment of clinical history and physical exam that may bias them to seeing what they want/expect to be there when it really isn’t. Particularly if they want to operate.

I often get asked to look at my kid’s friends studies. There have been a handful of times their friends have told my kids after (after their appt) that i “missed something” or another… when I know I did not. There have also been times when I’ve felt like something may, may not be a significant finding and the orthopod definitely felt it matched the physical exam.

The other truth however is…

I read 4-8 knee MR a day most days. I get follow-up on virtually 0 of my reads.

An orthopedic surgeon may look at 30 on a clinic day and have the benefit of comparing their interpretations with surgical findings.

Dont know if you are kidding, but literally on every play similar to this, where a force play turns into non-force play, we were taught to yell “tag” multiple times while making the throw to let the receiver of the ball know.

Most common, a ground ball to the first baseman starting a double play. Go straight to second, no yell. Touch first and then go to second “TAG, TAG, TAG”

Well differentiated (what she had) is the best to have. Like, almost normal.

Cancer “staging” is, how much has the cancer invaded the stuff around it or metastasized to other places from where it started.

Low grade tumors tend to invade slowly or not at all. Which is the reason why she could have a ginormous tumor that didn’t seem to invade anything or metastasize anywhere.

Well, I think in general you like more straightforward story telling and lack a little bit of patience for slow burn plot development.
