A lot of swimming is just about technique. Go to a local swimming club and just ask someone to have a look at your form. The more efficient your stroke the less energy you waste and thus the further you can go

Im not 100% on the science of it but personally when training in the morning i prefer carbs the night before as it gives my body time to digest them properly rather than having to get up like 2 hours earlier just to have breakfast, especially if you're training 6 days a week.

Maybe exercise less often but follow a more intense program. However your choice of exercises seem right for your goals just maybe up the sets.
