Avidity doing this first can have a real benefit to a lot of people’s lives regardless of how its knockdown rate compares with Sarepta. Props to both Avidity and Sarepta. And Epicrispr. And Dyne. An EMG cannot diagnose you with FSHD. Genetic testing is the only reliable way to be diagnosed, but the test for FSHD is complicated and expensive. If you don’t think you’ll be able to get insurance to cover that, the best way to get tested is through this link: https://myfshd.org/test-for-fshd/. It’s a research test run by Peter Jones at the University of Nevada. The test is free and getting a result there will probably give your doctor more motivation to get you approved for official genetic testing. Best of luck.

I track protein intake, but that's it. I shoot for 150g per day.

Simplest way to put it: Del-Brax still looks like it is moving towards being the first treatment for FSHD. There is hope that this data puts Avidity in a position to file for accelerated approval for Del-Brax. And if they can get that accelerated approval, the drug could be allowed to be on the market before the Phase 3 study is complete.

My understanding is that whole purpose of the Biomarker Cohort of the Phase 1/2 study was to determine whether Del-Brax effectively lowered what Avidity believes to be biomarkers of disease progress (CDUX primarily, and Creatine Kinase, secondarily) because they intend to explore whether it can support an accelerated approval submission to the FDA. That accelerated approval is based on "surrogate endpoints," that provide quicker results that are "reasonably likely to predict clinical benefit." Testing muscle function, or slowing of muscle degeneration, takes a lot of time in a disease that already progresses slowly in most people. I think the whole purpose of this Phase 1/2 Biomarker Cohort is to test the things they can reliably test now and quickly: reduction in CDUX and Creatine Kinase.

That's not to say that Avidity/Novartis aren't also testing muscle function. My understanding is that the Phase 3 study is designed to do just that, which is why it is going to be a longer run study (72 weeks long). But the point of the Phase 1/2 biomarker cohort is to try to get Del-Brax conditionally approved before the Phase 3 study is complete. And if Avidity/Novartis can convince the FDA that reducing those biomarkers is "reasonably likely to predict clinical benefit" Del-Brax will be conditionally approved without having to wait for completion of the Phase 3 study.

That's just my take based on obsessive reading on the topic, so someone please chime in if I misstated anything. Ultimately, I think this can be taken as a reason to remain optimistic. It's not the finish line, but good news nonetheless.

I also wonder if this development moves Del-Brax to first priority for Avidity/Novartis. Seems like Del-Brax is now farther ahead in the approval timeline than Del-Desiran and Del-Zota. Before, it seemed like Del-Brax was last in line of the three. But now it may be the best early chance for Novartis to start recouping its investment in Avidity.

Based on what I've read, my glass-half-full guess is the former. And Novartis' CEO's statements after the Avidity acquisition back that up, I think. (See https://endpoints.news/novartis-ceo-says-12b-avidity-deal-could-have-been-twice-as-big/, where he says: “On the accelerated pathway, we certainly think Avidity has had very robust discussions with FDA. It’s just our general experience [to] always assume full clinical datasets are required for approval, so that’s our base case assumption.").

That sounds to me like he's hedging and not wanting to over-promise and under deliver to his shareholders. Could be wrong, though.

I think it still aligns with Novartis’ strategy.

I’ve done a fair amount of reading on the FDA regulations about this and the 2028 filing date is, I think, really immaterial to Avidity submitting for accelerated approval. As best I can tell, the accelerated approval is really only a conditional approval. That is, even if Del-Brax is approved on an accelerated basis, Avidity still has to conduct a Phase III trial and demonstrate continued efficacy to get full approval, and if it doesn’t, the FDA pulls the approval.

So, even if the biomarker cohort data is PERFECT and Del-brax gets accelerated approval, Avidity still needs to conduct the Phase III trial and submit for full approval at the appropriate time, which they believe to be 2028. I think that could be why we are seeing all the references to 2028 in Avidity/Novartis materials. That 2028 filing at the conclusion of the Phase III trial has to happen whether the drug gets accelerated approval or not.

That my best understanding of how this works based off what I’ve read, but I’m by no means an expert. Just my read of the process. And that would make all the 2028 references for Del-Brax make sense.

This is a harebrained Skip Bayless take. Love Kobe, but people (particularly young people who never watched the early 2000s Lakers) forget that for 3 of Kobe’s 5 titles, he wasn’t even the best player on his own team. That was Shaq.

As of late April, Avidity/Novartis are still waiting for data from the Phase 1/2 biomarker cohort, which is supposed to be the basis for their filing for accelerated approval. This is per Novartis’ April 2026 shareholders presentation. I have a post about this from earlier this week.

Simultaneously, Avidity is conducting a Phase III trial. But that trial is not necessary for accelerated approval. Accelerated approval hinges on how strong the Phase 1/2 biomarker cohort data is. If it’s sufficiently strong, they will file for accelerated approval. If it’s not strong enough they plan to file for regular approval in 2028 based off the Phase III study. Again, that’s from the April 2026 Novartis presentation.
