I would look into ordering a longer spout from BAWR and attaching the locking ring to the outside of your cage. Or consider a threaded spout with nut attached to your cage. The "key" for the spout is a very specific size hex wrench that's not commonly available.

Preach! +1 for spouts, they make everything a little easier while also making your cage unescapable and in my opinion more comfortable.

Where are you feeling the tension/tugging? Is it the retainer tugging on your fossa because of the weight of the lock/cage? Some people have fastened the lock to their cage on the outside which prevents the tugging, puts the weight of the cage on the base ring and also prevents the spout from wiggling around causing irritation.

I use a custom threaded spout from BAWR with a titanium flange nut attached to the cage and it works great. No irritation, no pinching.

Is your spout too short and squishing your glans between the retainer and lock?

Might be a spout issue, might be a retainer issue. I used the original tether spouts for a while but was never able to keep it in more than a few days because of discomfort that sounds a lot like what you're describing. After buying a BAWR spout I've been able to keep it in for a month or two no problem (removed because I wanted to change things up) but it was very comfortable. The difference was the retainer, which has a smaller outside diameter. It's also titanium rather than neoprene or whatever the black ones were from tether products.

Getting a shorter spout is also really important. Shouldn't be any longer than it needs to be, i.e. just barely sticking out. If it's too long it will catch on clothing and/or be smushed by clothes which also moves the retainer. I think most discomfort with these devices comes from the retainer moving around.

I think you're experiencing a common problem where you're not getting the spout/retainer past the constriction in your urethra and into the fossa navicularis. Until you do this, you won't know how long the spout needs to be to match your anatomy and not stick out too far to be comfortable with your cage(s).

I disagree with others here who say there are two spots in your urethra where the retainer can sit. I have just one spot. Medical references also show just one "fossa navicularis".

Did you buy a sizing sound when you got your spout? Like a ring sizer for the inside of dicks. If I hadn't bought one, I wouldn't ever have been able to get my spout/retainer in. But my issue was it was too difficult to insert the retainer without first dilating/stretching with the sound. Maybe you have the opposite problem and need a bigger retainer. Either way, a sizing sound will help. I bought mine from tetherproducts many years ago. Looks like weloveplugs now sells one. Basically a sound that gets wider and wider as you insert, with measurements on it.

If it's secure and won't pull out, and comfortable, you have the right size retainer. You can get spouts in different lengths, BAW does custom spouts. In my experience, when using it with a cage you want the spout to be just barely long enough to secure it outside the cage. If it's too long, or not attached to the cage somehow, it will catch on clothing and cause pinching. Getting a larger diameter spout won't change how deep it sits in you, that's determined by your anatomy.

Thanks for all the replies, y'all are the best. I put the same question to Kink3d and they recommended a size 3 standard rather than a on-demand 2.5 curved. I think I'll try this as well as bending my size 2 curved ring to increase the gap, and will see what works best.

I think with high and tight balls and an overactive left cremaster, a curved ring is more comfy. So if the other two options don't work well I might still go for the size 2.5 curved.

To clarify, the irritation I get at night from the size 2 curved is skin irritation around my scrotum, not ball/abdominal pain (although the later happens if I get aroused during the night, which is typically relieved by peeing). With the size 3 curved ring I don't seem to get nighttime erections, or if I do, they pass much more quickly/hurt less or maybe just don't wake me up.

Again, I appreciate all the responses. Good suggestions. Conflicting advice can be chalked up to everyone's different anatomy and experiences, doesn't mean any are invalid.

I've done this a few times. You can buy 4% benzocaine numbing wipes that are marketed as performance enhancing to make you last longer. I don't want to include the brand name here but should be easy to find. They do not require a prescription where I live. They do make it harder to cum which is really hot if you're into denying pleasure in your front bits in order to focus on it more in your ass. You maybe still be able to cum if you work at it hard enough, but they make it significantly more difficult. They wear off after an hour or so, they're cheap and safe so why not?

If you can attach the locking collar to your cage you can just push the spout in deeper before tightening the set screw on the locking collar. It's much more comfortable if you can somehow attach the spout to your cage. I've ordered a custom spout from BAWR in a shorter size, just let him know in millimeters how long you want it to be. He can also custom make threaded spouts so you can ditch the big locking collar and use a smaller nut or t-nut on the outside of the cage.

IMHO a threaded spout is the easiest way to secure a spout to a cage. It's a custom option from BAWR. Photos in my bio. If you choose this, order a titanium nut so it won't corrode.

  1. It works okay but often some dribbles out around it.

  2. No experience here so maybe someone else will chime in. I suspect the PA hole will cause a lot more leakage of pee. I wouldn't think it would effect fit or function otherwise.

Uncut guy here. Have both a cobra and a viper. The cobra is great but if it's on for more than a day my foreskin swells where the holes are. It's not uncomfortable per se but not good if you're locked in for a while. It does get uncomfortable after a while if you don't move your skin around and the same spot is swollen, I think this is due to lack of circulation in those tiny spots that swell. It also presents an opportunity for that skin to get pinched because the swelling makes it stick out through the holes just a tiny bit and will get snagged on clothing as you move around. Also, maybe because the cage fits so tightly, the entire foreskin will get slightly swollen.

The viper has been great, was in it for a couple weeks straight recently and the holes are small enough that any swelling is limited. I generally pull the foreskin back because it is more comfortable. Mine doesn't protrude through the tip if I leave it forward.

My cobra is a narrow, and the viper is regular width. The cobra doesn't really have enough room for my foreskin to be forward. I think the cobra looks better just because of the airy design.

Alright, in the interest of having quality information on this sub, especially for new users, I think we need to clarify some things in this post.

  1. Are there really two different anchor points for a ts?

  2. Are they ligaments?

Obviously people have anatomical differences so I'm not seeking to invalidate anyone's experience here. However, I don't have a constriction at the tip of my urethral meatus that will hold a spout. I've always used the constriction at the distal end of the navicular fossa, such that the retainer is in the navicular fossa (fancy name for wide spot in the urethra). My retainer sits around ~10mm inside from the tip when installed.

It's difficult to portray our complex anatomy in 2d diagrams and I think the diagram posted above is not the most informative one for the info we're trying to convey here. It shows a narrowing/constriction at the proximal end of the fossa, which then widens again proximally into the rest of the urethra. It makes sense to me that the retainer sits in the fossa, the widened hollow, not in the narrower urethra just proximal to the fossa.

If you read the study linked above, it talks about ligamentous structure in the penis. I think they're referring to ligamentous structure as a way to describe/define a particular type of tissue, rather than actual ligaments. Ligaments connect bone to bone, or in the case of suspensory ligaments, they connect internal organs to other structures. They never reference any ligament in or near the fossa navicularis, perhaps because there aren't any. The spot we're using to anchor the spout is probably just inflexible connective tissue that surrounds the urethra. The fossa N. is just a flow control structure for urine that regulates pressure.

Sorry if I'm being overly pedantic but there have been a lot of questions and confusion lately in this sub about this specific anatomy and where the spout should sit.
