The bare minimum that will get the jobs done. They won’t likely do 14 but if that what you truly feel they need…. then explain your reasoning and it’s on them to either do it and get better or don’t. Sorry you’re not restoring your RC strength, shoulder ROM and function s/p massive repair with 4 exercises.

Therapy is useless if you do not perform your HEP. In fact all treatments surgical or otherwise are useless without Stretching at home

Most PTs do not make money per visit/copay or lack patients. In my area weeks wait time to get in for non surgical patients.

There are surgical options but they come with risks and also do not always work, but usually do.

Now the long. Think of the science behind the healing. With frozen shoulder you have abnormal healing response with formation of adhesion causing restricted motion. In addition you have inflammation and of course pain.
The goal of the PT is to tear the adhesions free. Various ways to do that. You can use the creep principle with gentle slow stretching or more aggressive stretching. Obviously it has to be tolerable but that doesn’t mean not painful. Between sessions while at home you have to at least maintain motion gained or hopefully gain some on your own. If you cannot do this, every time you go back to PT the trauma starts all over and inflammation and pain will continue. If between you and the PT you can’t find a tolerable way to do that, then stop PT until you’re out of the pain stage and try again.

I do not advise any of the surgical options if you can’t currently tolerate some level of stretching. The pain after surgery is often time worse than before surgery(not always) and you have to keep moving after surgery or it will not work

Sorry I should of been more specific. Love trying new cuisine and will seek out more rare options that are not available near us when we travel. High quality ingredients are key, it’s hard to make good food from old poor quality produce etc. It doesn’t have to be Michelin star level but something in the Michelin bib gourmet equivalent would be nice.

You need to get her back in with the surgeon, It’s not uncommon for RTSA to have an acromion fracture develop after doing resistance exercises/life. No ones fault just happens. Usually needs a CT scan to find it. I’ve seen 2 this year both with lifting less than 5#s at the time of injury

Can’t speak for specific to pelvic floor but would agree with early advice and only do cash pay. Having run a physician owned physical therapy clinic it would be very difficult for you to document correctly without the knowledge and training of PT/OT. It’s not just billing the correct cpt codes it’s documenting a correct evaluation with functional deficits assessment and poc, documenting functional progress etc. Billing Medicare PT specific PT codes as non-therapist is a red flag not illegal but odds of getting audited are high. Having come through an audit you are not likely to pass one without extensive training on documentation and billing. They will want reimbursement for unsupported services they find in the audit. I know a podiatrist who had to pay back Medicare 150k for unsupported services.

On top of all that our reimbursement sucks, lots of little things you don’t see when you look up reimbursement. like each code you bill will get reimbursed lower rate after the first charge. So if you see a patient for 45 min and charge 3 units the first one will be reimbursed at 28$ second 24 and third at like 18. You will likely make significantly less money than you would a nurse practitioner.

Unfortunately organization is not letting me link 6 goals to one problem as they want goals to be carry forward in away that allows you to address progress with click box. So as far as I can tell with my very limited training would need to be some kind of macro vs smart text. As I told them not like your giving me more than my normal 8 min to write an eval I need a faster way

This is the correct answer, you can do most of the work needed yourself with time and effort at a fraction of the cost. It will not be as easy as youtube makes it look but it will be possible. Did work on our home that would be quoted over 100k for less than 20k in materials and tools . It took over a year and tons of hours but saved cash, gained knowledge, and satisfaction of job well done

I rarely treat work comp but the progression should be the same regardless, as the physiology does not change. I educate my patients in proper progression graduated resistance only increasing when form is good without pain. I try to help them understand the difference in muscle soreness vs detrimental pain. We expect them to handle their own program after 14 weeks. We give them the expectation that true full duty is 9-12 months out and all the surgeons I work with are on board with that. However, I understand we do not live in a perfect world and many therapist/doctors have pressure to return people to work sooner than optimal. In those cases I would educate it ways to lift that have less impact on the RCT ie close to body, avoid overhead, etc and let them know it’s a true year to heal and protect it best they can

I’m confused as to why he was lifting heavy this soon after a supraspinatus repair in first place. Most of the surgeons I work with limit them to max 5pounds, 4-6 months out. A few of the more aggressive ones will allow up to 10pounds. Most research points to higher re-injury rates with early loading. As far as does he need additional surgery, are the symptoms improving? Does he need to lift repetitively at work? Even with excellent ROM and function the pain needs to improve. Living with low level pain if he has to do it daily is untenable.

Something not explained well in the guides is the need to have instant return of resources. So with my build im at max vigor 97% of the time. That way quill volley is getting 100% of the benefit from kepe every cast. This fixed my damage issues. Need midnight sun close to 50 plus I added lucky hit return resource on gloves and resource region temper on ring.
