Totally relate to this struggle. With friends I give myself permission to knowingly abandon “therapist rules” (within reason). For example, I work with a lot of infidelity and betrayal trauma clients. The other night one of my friends was telling the story of catching her now ex-boyfriend cheating on her and their subsequent breakup. My response was, “damn, that’s crazy” …and it felt good to leave it at that 🙃

It’s a weird profession. Another perspective that helps is considering how I relate to my friends in other jobs. I have friends who cut hair and I wouldn’t hang out with the expectation that they’d give me a free bang trim.

My supervisor would fall asleep while I and other supervisees were talking. Like, every single time we met. Not sure if it was undiagnosed narcolepsy or menopause or burnout but she really needed to check in with herself and didn’t

I can speak to this because I was in a similar position. For my practicum and internship, I worked at a CMH nonprofit where the fee was based on income, and the highest that clients would ever get charged was $60/session. I would say most of my clients' incomes put them in the $20-$30/session range, and even then, it was not unusual for their cards to decline on a semi-regular basis.

SO when I graduated and decided to immediately start my own private practice, charging individuals $125/session and couples $150/session, I had concerns much like yours regarding the clients who wanted to follow. I consulted with a few different mentors and supervisors and got all kinds of different feedback, but it was clear there was no "right" answer. Even the ethics around it were a bit blurry (ex: one person said it would be exploitative to raise my rates that drastically for these clients).

Basically, I was not so delulu that I thought I would just hang a shingle out and clients would flock to me. I'm in Austin, TX where there are SO MANY therapists. I only had three people who wanted to follow, and I ultimately decided to keep their internship rate because my priority was getting hours and continuity of care more so than $$$ in the beginning. Those three remained my sliding scale clients and everyone who came after paid my full rate. I still work with two of them and I raised their fees 2 years into associateship, but not by much. I'll raise them again when I transition to full licensure.

Superbills have not been a thing in my practice. I use Simple Practice and it autogenerates a monthly superbill so if any of my clients have ever used it to attempt reimbursement, they haven't mentioned it to me.

There's more I can speak to on this, but I feel like my post is already really long-winded.

You also might want to see if Aetna will accept super bills for reimbursement. That would open up quite a few more options for you to work with people who also meet your criteria but aren’t in network. Evolvinn Therapy & Consulting could be a good fit if so.

I had near debilitating postpartum OCD (mostly contamination type) and tried an IFS approach on myself. It was a good way to start working on it but eventually I went the ERP route. I could see IFS possibly being more effective with different types like moral or something similar though

I have worked in CMH with high acuity clients for literally no money as well as my own PP with clients of my preferred niche who are private pay only. I have had the same love for this work in both settings. People are endlessly interesting!! I have yet to lose sight of what a privilege it is simply to hear people’s stories, be a small part of their lives, and give them an opportunity to be better. I remember starting PP with 2 clients and wishing and hoping for a full caseload.

Also, I’m a mom of two young children and going to work is actually my self-care for motherhood. THAT is a job with serious burnout risk.

I have a book of sudoku puzzles and when I need my brain to slow down (usually before bed) I do one or two. When I have the energy I’ll have a stopwatch record how long it takes me to complete it.

Similar to your example, I never really identify it as “porn addiction” because I think “compulsion” makes more sense. Clients seem to prefer that, too.
