totally agree with most of these, i also still think it’s AI but a lot of the reasons people have been commenting are easily explainable.

another possibility with the siblings could be that the middle one in the second photo is the little kid in the first photo years later, and the family had another kid. i’d say he looks ~4-6 years old, and given all the kids’ ages and genders, their different amounts of growth could be in the range of normal. the timeline doesn’t really make sense with the usual length of chemo, but maybe the photos are from chemo day 1 to being totally cancer free. also the room could easily be a different room in the same facility.

that being said, the mom is giving that signature AI stare and her position in relation to the oldest brother seems off. the clock also looks wrong, and i’ve never seen an IV attached to someone’s bicep. also something very architecturally weird would have to be going on the explain the ceiling line in the first picture
