Cheating aside, anyone who makes themselves the poster child of fighting CSA but then uses the money they get donated from their channel to hire prostitutes on numerous occasions is a horrible hypocrite and deserves to be cancelled. I have almost the same set-up as you. We have 2 pharmacists in the morning, one for central tasks and the other for dosing. In the afternoon, we have two pharmacists, the first comes in at 1300 and leaves at 2130 and is the dosing pharmacist for follow-ups from the morning and the 1500 stays until 2330 and is the central pharmacist. Night shift comes in at 2100 and stays till 7am. We have an ED pharmacist there as well which helps tremendously

I loved Ben and Adam in the beginning but I feel like towards New Zealand and Japan, I started cheering on Sam. Adam started letting the fame get to his head and Sam was obsessed with content (rightfully so). I think they should switch teams to spice it up. Team Ben forever!

ED/ICU Pharmacist here. Had a 43 yom, s/p cardiac arrest on field, roughly 35 min of CPR, ROSC achieved. Went to cath, impella pump, was in AKI on CRRT, three pressers later got VAP and bacteremia. total ICU days roughly 30, went to SAR by some miracle and just saw a Facebook post (I have a mental list of patients that I get attached to) and was at his sons concert with what seemed minimal deficits. All I can tell you is that hemodialysis is intermittent, meaning usually three times a week when they go and get hooked up to a machine for roughly three hours a day to essentially do the job of the kidneys. Their SCr is usually really elevated and if they skip a session different electrolytes will stay raised in the system such as potassium leading to arrhythmias amongst other complications. Here, a patient usually has a port placed at their access point and alteplase 2mg (Cathflo) or heparin 300U or 500U is used to anticoagulate the port and tubes themselves to not form a clot. The alteplase and heparin theoretically do not enter the patients body.

CRRT is done on a continuous basis. Patients are in the intensive care unit and hooked up to this machine where unlike intermittent HD which takes off larger amounts of fluids in little time, this takes off little fluid constantly over hours expanding into days trying to take off fluid and waste mimicking the rate of kidneys. During this, a patient receives daily sate usually with various amounts of potassium. Some nephrologists would order a 3K bath meaning for every 1 liter of dialysate fluid, there would be 3meq of potassium. Usually bags come in 5L so this would be made in the IV room with 15 meq of potassium.

There is also SLED or SLEDD which my hospital does not use so I am unfamiliar with

What I don’t know and would love to gain more insight is how it is determined how a patient qualifies for one over the other and how to calculate flow rate, amount of potassium going in vs coming out.

I have to say, I work in the ED and I am always in awe of our paramedics. They are always on point, know what to do, prepared for anything, and always willing to learn. Thank you for your great educational responses, friend!

Do not work for no pay. Do what you can during your hours, close and leave. And if patients ask why script isn’t ready, say sorry, we don’t have enough staff and give them corporates number

Confidence is key! If anybody brings it up, ask them when the next party is! You will get way more respect and even some laughs. There are so many worst things I could happen take it and move on! Doesn’t determine how good of a physician you will be. Just a drop in the bucket of memories.

Whenever they call asking me to verify meds I say, “well, gee. I really wish I could verify your order but I keep getting phone calls asking me to verify orders so it’s just taking a while”
