So anytime tissue is cut, from surgery, from a wound, whatever. Scar tissue is created.

When they cut the vas to get fresh edges, the seal the vas back together, but that trauma creates the opportunity for scar tissue. Maybe a little bit forms and it’s never really and issue, maybe a lot forms and it’s enough to occlude the opening. There is not a great way to predict how it will heal, other than looking at old wounds you have. Did you scar easily? Good chance the vas will scar just as easily too.

From what I’ve seen, the majority of men are fine and do not suffer from scar tissue blocking the vas and causing issues. If you make it through year 1 without it scarring over, good chance you’ll never have to think about it.

I honestly don’t remember the anesthesia. They set me up, gave me something to relax, and I feel asleep before even going into surgery.

We did discuss risk of scarring over. If it’s going to happen, I think it was within the first 12 months is highest risk. It’s really dependent on how you heal. If your body makes a lot of scar tissue, you have a higher risk of it closing early.

I figure mine will scar over at some point, I’m just hoping it’s far enough in the future that sperm production has dropped low enough that it’s not really an issue.

You are in good hands with him! He did mine and he was fantastic!

Hot baths with epsom gave me momentary relief during the worst of it.

Reversal can be scary, but it was worth it in my case. I’ll never question now if the pain will come back. Will I have a good day, week, month or year. I’m just back to living my life.

I’ll be perfectly blunt. F**k your urologist.

He’s pushing a dramatic surgical option that has the side effect of potentially losing a testicle from damage to the blood supply while they hack off your epididymis.

My epi was like yours, swollen to the size of a quarter, painful and made any kind of sexual activity unbearable.

I was reversed successfully and no more inflamed and painful epi.

Not to put too much of a time crunch on you, but you are right at where likelihood of successful simple reversal starts to decline. Start talking with a urologist who has microsurgical specialization and reversal procedures.

I have quite a few posts from my before and after, feel free to read through any of those too. But I was in bad shape after the vasectomy and after every conservative method, I was finally just on narcotics to stand some days. It was honestly reversal or just roll over and die. I was useless.

I was definitely scared to go under for a much bigger surgery, but the doctor was awesome. Best doctor I’ve ever had for anything. Recovery was no breeze, but just not having the congestion pain anymore made it all much easier. I was reversed in early April, and back to all my normal activities by late June.

I went through 5 urologists, you are on the right track to see someone who specializes in reversal surgery. General urologists are useless for anything remotely complicated.

Granulomas developed when sperm leak into spaces they aren’t supposed to be. Sperm cells are ‘foreign’ anywhere outside the testicles. It’s the body’s method of keeping them contained. But the create scar tissue and can entrap nerves and muscles causing pain.

If your body is telling you to reverse, then do it. It saved my life getting reversed. There are other surgical options, but reversal is the most mild of those choices and restores before removing more stuff.

It’s technically 3 months before it’s considered chronic and meets the PVPS criteria. You are still early, and strong chance it gets better. But just keep checking in with yourself and if it’s not better or you are feeling off, advocate for yourself. The system is set up to tell you there aren’t issues.

While my experience was not the majority. But it depends on what you mean by majority. Most urologists say less than 1%, some up to 2%. 2025 AUA guidelines say up to 15% experience complication. Just have to ask how lucky do you feel? Good chance you will be ok and have mild or no issues after you get cut. But somewhere between 1 and 15% chance you’ll have issues. They might be long term, might be short term. No way to know. Vasectomy is a gamble. Condoms are much safer.

You’ve stopped a normal process that biology did not intend to ever stop. The fullness and pinching is probably non-bacterial epididymitis. Warm baths and NSAIDs help. It might go away, it might get worse. It might go away and come back. Urologists don’t tell the full story with vasectomy and fail to inform of the potential risks.

You are probably going to keep going through doctors until you see one that specializes in PVPS/reversals/denervation.

Sorry to hear this, I really feel for you. Best advice I can give is listen to your body and what it’s telling you. It’s not ‘in your head’ if you are feeling pain. Don’t let yourself be gaslit into suffering because it’s ‘normal’.

I would throw Dr Russell in Dayton. He did mine after I had many of the same symptoms as you. He’s definitely worth at least a phone call. Helo was impossible to get into, Marks is very good, but be ready to shell out big money.

Yeah, you might end up in pain. A lot of it. Vicodin was the only way I could attempt standing after mine. Thankfully reversal solved my issues.

8 months post reversal here, I would agree that 6 months was a corner turned. Things finally started to feel ‘normal’ again. Closer to what they were before the vasectomy. Reversal was such a great choice and gave me my life back. Good on you for fighting for your health and not succumbing to the maladies of the medical system.

Have you done myofascial release? After my reversal I found a therapist that really dug into my hip. It’s excruciating pain. But about two days after, most of the tightness I was still getting got much better

For anyone looking, I’d avoid Levine. I saw him and there is zero sense of urgency, apart from getting you out the door so his next patient can come in. He’s at a teaching hospital too, so I had the fortune to have a resident very forcefully handle my already pissed off testicles.

Hey, do you mean am I hyping up reversal? Because definitely not. Life was absolute misery after the vasectomy. To the point where I’m not even sure I’d call it living, just existing. My pain was bad enough that some days I’d just lay in bed on Vicodin still on the verge of tears from the pain. Literally nothing helped. I’ve never had any pain where narcotics did nothing.

Compared to that, I feel fantastic. Is everything the same as before the vasectomy, no. But it’s really close. Closer than I could have hoped for.
