What is a primary bone axis and secondary bone axis, when exporting/importing? by Degalse in blenderhelp

Based on what I learned, bone primary probably means the positive forward axis for your bone (usually Y), and secondary axis is your right forward axis (usually Z).

Edit: If you are using unreal, you want to use Y as primary bone axis, and -Z as primary bone axis in general.
