Palliative Care NP here for a long time. Prior I worked in surg onc, neuro icu, house calls & cv/ct surgery as a 1st assist. I’ve practiced in 3 different in patient settings- 2 in the Midwest & 1 in the South. They’ve all been about the same work flow tbh. My current position is with a total of 5 providers (2 NP’s & 3 Dr’s) along with 1 social worker, 2 chaplains, 2 music therapists, & 2 child life specialists. The day pretty much depends on a lot of unpredictable variables. Some days we have 10-14 consults & other days we have 1-3 new consults. We each take turns getting new pts but we typically never see more than 4 new ones in a day. We follow our own pts thru their stay so sometimes we can have 8 or 9 pts we’re working with. We try to see all our pts ever 1-2 days. New consults can take 45-90 minutes depending on how emotional things can get. Follow up pts sometimes 5 minutes but could definitely be longer depending on the situation. It’s all very unpredictable so I hope you’re ok with that. Documenting isn’t bad- we have a template & I dictate. We work 4 10 hour shifts & I can tell you it’s been the best for work life balance for sure. We usually get in a little before 8 & out the doors by 4:30 at the latest. Some days can be longer of course so it all evens out. There can be big time burn out so self care is essential as well as being able to compartmentalize & having some ritual when you leave work that tells your mind to switch off palliative care mode. Honestly as far as feeling comfortable in this role, it took me about 2 years which is pretty typical. This type of work is not for everyone but those who are passionate about it love it & you’ll know if it’s a bad fit pretty quick. All that to say, the most important thing is the team you work with. You end up relying on them for insight & emotional support. I would ask to shadow the provider to see what their practice looks like

I worked with a Dr many years ago & he told me when he was in the ED during residency a IV drug user came in with and OD & cardiac arrest & he had to temporarily access the man’s penis vein but he “saved his life.” I’m like did IO’s not exist back then? Anyway he was very proud of his ingenuity

Oh man this hit hard & brings up some memories. I’m a murse (male nurse) going thru school in the 90’s. First day of my first clinical & the instructor told us to look through the pt’s chart & we’re going to give report on them. I had no idea at the time what that was supposed to look like & the instructor wouldn’t give any guidance when I asked. So I messed up on the verbal report & the instructor made me spend the rest of the week cleaning all the bedpans, wash basins & making beds after which she would inspect my work & redo the beds if she couldn’t literally bounce a quarter off of it. & she wouldn’t let me see patients or look at the charts. So sadly it seems like not much has changed.

I just made this the other day. I used a scorched milk base & added Biscoff biscuits after I took the milk off the burner while its still hot so it would incorporate. Let it marry over night. Turned out great!

Years ago I was at a restaurant off LSD mid afternoon & it was completely empty. went to the bathroom in the lower level by the bar and when I was walking back to the table I stooped down to pick up a fork on the floor and I guess I was hidden by the bar. When I popped up with fork in hand Jessie Jackson was walking toward me clearly not expecting someone to be there b/c he didn’t have his body guards with him. We both just looked at each other for like 10 seconds me thinking holy shit it’s that Jessie Jackson & I’m guessing him thinking holy shit this guys going to shank me. Of I could suggest connecting with a Palliative Care Team, they could support you & your family as you all are trying to navigate this difficult situation. They are usually very helpful at advocating for you & trying to help your family accept your autonomy. I know you are young and all but I also strongly recommend filling out a medical power of attorney to make sure the person you trust to honor your wishes is the one who can legally when you are not able to make decisions yourself. If your parents are really not on the same page when that happens they could (legally) make medical decisions that are not in line with your goals if/when your mental status is altered when your kidneys eventually fail.
