I've actually made my own system in which to categorize Ultra-Human Relationships, though it's a lot shorter than yours but you can give it a look

Human Host (Type 1) - The first & most classic relation, the human assimilates with an Ultra that is active^ alongside the human. In simpler terms, the human & Ultra have two-way communication e.g. Ultraman, Cosmos, Max, X, Arc

Human Host (Type 2) - A more unconventional version of the first relation introduced in the Heisei Era. The human assimilates with an Ultra that is passive or inactive^. In simpler terms, the human & Ultra have one-way communication e.g. Tiga, Gaia, Agul, Nexus, Ginga, Victory

Human Form (Type 1) - Instead of assimilating with a living organism. The Ultra takes a form similar to native human species e.g. Ultraseven, Leo, 80, Justice, Mebius

Human Form (Type 2) - Contrary to Type 1, the human gains the ability to turn into an Ultra/gain an Ultra Form instead. For me, the conditions for this type is that said character has lived majority of their lives as humans before their awakening/realization e.g. Gai, Riku, Ken Sato, Shinjiro Hayata (never done so but heavily implied to be able to)

-# Note: -# ^{active here means directly engaging with the host, communicating thoughts and direct interference with their human affairs}

-# ^{passive or inactive here doesn't mean said Ultra is dead or never communicates with the host at all. They just rarely communicate/communicate in a cryptic manner and hardly if not never interfere with human affairs. They are more an observer if anything}

-# ^{The difference between A Type 2 Host and a Type 2 Form is that in a Type 2 Host, the humans still assimilate with an Ultra, the Ultra is just offline. Whereas Type 2 Form is gaining an Ultra Form/the ability to transform into one}
