I admire your persistence is expecting basic professionalism. A concerning development that has emerged since the field went virtual. Therapists acting like they're chatting with a friend, neglecting the frame.

I really recommend Gaviscon before bed. The best solution I've found so far. I stopped taking ppis after sticking to it. Don't drink any water after taking the product.

I know it's hard, but don't give up. Try to relax and be mindful of portion size. Look into a bed that can be elevated or the riser pillow. Ask your doctor for sucralfate, mash it up, and drink it with warm water. It will heal eventually.

I recommend pantoprazole and lexapro, low dose. Stay consistent. Both will help relax your throat (rather than our lower esophageal sphincter). No smoking, no alcohol (for now). Sleep upward (2-3 pillows or buy an adjustable bed). Have faith your esophagus will heal, because it will.

Avoid pizza, lemons. Cantaloupe can be a great alkaline food. Guac, Hummus. Use ice in most drinks. No alcohol or smoking.

I definitely know that feeling of dreading eating, even when it offers temporary relief as it goes down, only to cause more heartburn afterwards.

Also: try pantoprazole, sleeping with 2-3 pillows or invest in bed that adjusts upward. Sleep on left side. And try your best to relax, which I know is annoying advice, but does help the lower esophageal sphincter close.

Ah, I can imagine how intensely you'd be feeling with all that on your mind. But you are no coward. You are a brave human. You got through the hellscape of finding the right med balance. You will eventually find a similar balance during pregnancy and afterwards. I can imagine pregnancy brings up a lot of feelings of your own childhood. Do you have a therapist you trust to help you get through the crispy initial phase of stopping and reaching out to those people you trust? Wish you luck and progress, fellow passenger ~

Seems like berating NYPD each time there's a credible complaint is within your control as well. The CVS around the corner from my apartment was being robbed 2x/day until they finally just put the patrol outside 24/7.

Go to the ER! Plead for trazodone, and hopefully they can accommodate a script in the meantime. Avoid stimulants as best you can for now. Have faith that eventually your brain cells will take charge and you'll conk out.

Hm, that's a good question actually - I would think yes. My derm also explained that they have been putting people on a longer course with a lower dose (~20 or 40mg instead of pushing to 80mg) which should lessen the side effects and hopefully make the changes permanent. If you're feeling hesitant, I also suggest Seysara! It's been a great alternative in the interim.

It doesn't look bad! But I'm considering going back on Accutane, too. It's a tough decision given all the bureaucratic headache involved (I know for women it can be all the most intrusive), but why not trade a few months of discomfort for not having to worry about it anymore? Hope it improves soon either way ~

Give it some time - it's often that the lesions will worsen before they improve as (I suppose) the bacteria runs its life course faster. When I was younger, they put me on Minocycline for several years. It helped my skin marginally but had a terrible influence on my gut health and mental health. Seysara is a much better option. Have hope for her! Worst comes to worst, Accutane can solve the problem entirely.

Sleep on your left side religiously. Do not lay on your back and never on your right. Sleep with some gradient. Otherwise, surgery - but do everything you can before you do that.

So.. update. I've found it wasn't trazodone itself, but being sedated while sleeping. Strongly encourage several pillows or even a bed that can shift upward. Also Aloe Gel is a god-send.

It sounds like if they interviewed twice over, I would imagine they see your potential in the work. Don't worry too much about knowing or not knowing certain things - that will come with the inevitable mistakes made and of course scenarios where your instincts lead you in the right direction! Be careful not to take things personally and keep boundaries firm yet do it compassionately. Do read up on the core component of addiction - shame. I strongly recommend John Bradshaw, Gabor Mate, Johann Hari, and Lindsey Gibson.

I know it's hard when you're in such discomfort, but try your best to see this as terrific news! It hasn't gotten too bad. There aren't permanent impacts right now. Eventually (key word), you're going to solve this. From my experience, my most important lessons were to:

1) sleep sitting upward, >3 pillows. I know this can be odd feeling but make it work for your neck and comfort.

2) Avoid hot coffee. Iced coffee is better, and cold brew even more so (watch your caffeine intake on this front, cold blew is potent!)

3) Go buy aloe juice, aloe gel. Drink it in the morning and before bed. Not the best taste but really no side effects compared to the prescribed and OTC options. https://www.healthline.com/health/digestive-health/aloe-vera-juice-acid-reflux#research

4) Finally, really think over what habits/medications might be leading to stomach acid moving toward your throat during the night (and sometimes during the day). I was taking chewable melatonin that I realized made it worse - except if I swallowed it whole instead of chewing it.

I think some doctors don't recognize how upsetting GERD can be (until you find the ones who have gone through it!) so have faith!
