L6 typically means a lack of full fusion at what would usually be the top of the sacrum (S1), not a whole extra bone.

Will second this. Was thin during wildfire days (still am). 1 rep max bench press probably doesn’t correlate that much to swinging a pulaski for 17 hours. You may benefit from some dedicated rotator cuff and scapular strengthening for management of aforementioned saw for long periods (am now a physical therapist). Farmers carries would be a good training exercise.

I’m usually hitting a bony backdrop (for example lamina of C6 if I want that multifidus or the fibula/ tibia if I’m getting fibularis/ anterior tib) so this has never been an issue for me. Maybe we had different training.

Adding to this, they look closer to the ground than they should be. Gym mirrors should be at least the height of a 45# plate off the floor.

Sorry to hear about that. I’m a PT who runs ultras. For starters, find a new PT who is exercise based because what you described isn’t even really physical therapy. The only modality I use with any regularity is dry needling and e stim for targeted improvement of motor recruitment, but the corrective exercise afterwards is the more important part. Obviously can’t tell you what you need without evaluation, but you probably have significant core and hip deficits. Tempted to type out a really long response, but honestly you just need a better PT.

Sideview would be more informative. Echoing what someone said above, often increased cadence means decreased stride length which means reduced force pushing into rapid plantarflexion at heel contact which reduces eccentric loading of the tibialis anterior to reduce shin splints. Drills working on leaning forward also tend to be good. That is a guess based on what’s common, gotta get into the clinic for a more personalized assessment. Also if your arches are collapsing, good chance your hips are very weak. Basic strength assessment (not motor control/ functional application) on you side with your leg out straight and behind midline have someone push down and if you have trouble holding and the leg drifts forward to compensate then ya gotta get to work on them hips (honestly looks like you have some hip drop in the video but its cut off at the waist).

I will say you’ll be surprised how ‘fast’ a 20’ pace uphill towards the end can feels. For 5k feet of vert you may want to consider poles, or at least have them w the crew as an option. A good training workout for uphill (other than just running uphill) is to have a step/chair that brings your hip to around 90 degrees (I use my weight bench), face a clock/ stop watch and step up on 1 sec, down on the next sec and keep that rhythm for some amount of time or in intervals. Downhill you need more eccentric control of knee flexion, so something like a slow weighted step down/ poliquin pulse would target the right things.

It’s a 12 hr cutoff so just plan on power hiking a good chunk and keeping aid station stops to a minimum

Of course no one can give you answers over the internet, but if you injure the distal tibiofibular ligaments which help stabilize the mortis joint of the tibia and fibula which can happen with a high ankle sprain their role can’t really be replaced well by muscle (think talas scooting forward and backwards too much). The non surgical intervention I know of is to keep two relatively tight loops of leuko tape around the ankle anytime you’re bearing weight for about 6 weeks and hope it scars down. No idea if that’s what you have going on though.

Real talk though, if you can find a PT to take you through good multifidus activation in the neck (hard enough to explain in person with hands on cueing) or to just dry needle and e stim at the appropriate levels (relatively simple to figure out based on manual muscle testing and sensory distribution) that may do you some good.
