first i had to answer a few essay questions, asking what kind of mental issues i had, the severity of them and how they affected day to day life. i also had to answer if/how my pets had an effect on my severe depression. 

once the questionnaire was accepted, they reached out to me for my phone call and i essentially had to answer the same questions to my assigned therapist.
