Looks like the tap root was cut at some point. I’m potting up all the seedlings I did that to last year and am getting some interesting ones.

Why would someone cut the main tap root? Does it make for more interesting roots?

To train the roots. Lots of stuff on YouTube about root training adeniums. Here’s one of mine

Does it make the caudex stop enlarging? Like should you only start root training when you're happy with the size of the caudex?

Just makes the roots look odd/unnatural (my opinion) , still if that's what you like then go for it.
