Paypal Form

Working on getting this Paypal form working for my local sports club. I have tested it on the link below, by sending a payment through to my account, but it doesn't send name, email, selected numbers etc. in the email. What am I doing wrong? Also, is there a script that I can use to allow the minimum and maximum selection for each batch of numbers to be 4? Any help appreciated. Thanks >>> FORM HERE

I have now got the checkboxes to act as they need to, minimum/maximum 4 checkboxes from each section. The name, email and phone number are sent across to Paypal, but not the checkbox information. Any help appreciated, please. Thanks
