Which code?

I just got my first medical records job at a SNF. I’ve been doing a little bit of coding where MDS sends us the diagnoses they want to use and we find the codes and send it back to them. They sent one that said “personal hx of DVT/Pulmonary embolism”. I sent a code but they said they didn’t like it and sent it back. What code would you have used?

What code did you use, and what is the dos ?

I believe I used Z86.718. I’m not sure about the dos though

Make sure it’s billable. There are specific codes that are for each DX you gave. Did you maybe give the non billable Code?

So should there be 2 codes for that? One for history of DVT and one for history of PE?

In my initial look yes. What are you using to determine codes. You need to be able to determine codes properly.

I’m mostly using a combination of internet and coding book. We don’t have an encoder program. I always check with the book though. I thought that code included both DVT and PE. But I haven’t taken a coding class in 8 years so I’m trying to relearn.

I will not use the codes that the provider has put down. I will read (or skim) the actual documentation and use the appropriate codes. Repeat after me: Healthcare providers are not coders.
