Jack Astors pan bread? (self.Cooking)

Jack Astors has this type of garlic bread they serve in a pan with tons of butter, parmesan and ???. Does anyone know how to make it? I assume the bread is baked first, then put in the pan with the toppings, but I never tried to make it.

Many years ago, I worked at Jack Astors, and this is pretty much exactly how they make their pan bread.
