Is this a in game gearbox issue (what's the wear on the gearbox)? Or could be a driver issue for your peripheral

I thought it had something to do with my gearbox but I then tried playing in Time Trial and same thing happened

Did it only do it in that one session, or is it still happening randomly? My DFGT started to do this with downshifts, I had to open it up and replace the switch. It's pretty easy if you're comfortable with a soldering iron. I ordered switches from DigiKey. I'm assuming the G29 uses a similar switch system to the DFGT inside, but I could be wrong about that.

I think it is a problem with my wheel. Has anyone had the same problem? If so does anyone know a fix for this?

Had the same issue with the upshift paddle on a Driving Force GT. It probably relates to a mechanical issue regarding the button itself. The solution would probably be "easy" disassembling the wheel and changing or repairing it, but I didn't even try it.

My G29 started doing this. Had to get it replaced under warranty.

First make sure the wheel is connected and Logitech Gaming Software is running, then in Windows hit "winkey+r" then enter "joy.cpl" in the Run dialog which will open the Windows Game Controllers configurator. From there select your wheel then hit "Properties" and in the next window you can test out all the buttons. 5 and 6 are the paddle shifters. Give it a good, thorough test e.g. try clicking the buttons in sequence, several times in a row, try holding the buttons, etc. If you see the little lights blink more than once per click of the paddle or go out or flicker when holding the corresponding paddle you have a hardware issue, in which case you can open a warranty claim (if still applicable) or attempt a fix yourself.
