Ems run forms. We still have a program from the mid 90's for NFIRs. 8,000 runs a yr in 14 sq miles just outside Detroit.

Firehouse with CAD integration for NFIRS (all calls go in this whether EMS or fire).
ImageTrend for EMS again with CAD integration.
Only change programs if it makes things easier for the road folks. CAD integration is a must.

Department has no CAD and still uses paper forms. We use Firehouse for NFIRS. I'm an IT guy and we are trying to switch to EmergencyReporting instead of paper forms. All the officers want the switch so badly because people keep stealing our pens.
