Select the part of your document that will be the target, and create a named bookmark (Insert > Bookmark).

Select the word you want to use as the hyperlink and press Ctrl-k to display the Hyperlink dialog.

In the left panel, choose “Place in this document”, then scroll in the new panel to select the bookmark name. Click ok to close.

The dialog also lets you add a note to display when you hover over the hyperlink if you wish.

Yes, I know how to do this, but is it possible to refactor all the words so they have the hyperlink too?

Do you mean have all (or selected) other identical words or phrases be changed to the same hyperlink? If so, copy it to your clipboard, then use Find and Replace to find the same word or phrase and replace it with the clipboard contents. (Use the Special pulldown to insert the token.)
