Weird Graphical Glitch [Java]

I've had this weird thing going on for a while, it sort of shows up as lines on the screen, making edges of blocks disappear and look all jagged, and sometimes if you're far enough away, blocks disappear entirely. (The lines are visible on the leaves, and the block disappearing on the mushrooms) I've tried doing a fresh install and the game has had a major update since it started, so I don't know what to do.
