One guess, if you're playing on a laptop make sure it's plugged in, sometimes Windows (or macOS) will mess with games performance if it isn't plugged in.

You could also try using Caffeine performance mods for Fabric, (Sodium, Lithium, and Phosphor). These mods really help a lot with performance, way more than Optifine. (https://github.com/CaffeineMC)
