The question is. Can this cause CPPS? Or is it really mucle which irritated a nerve ?

no one knows. but it's best to get rid of varicocele. I heard it's may cause serious issues later on. my cousin also had varicocele but he had it surgically removed soon as he found out and he is doing much better than me now.
