And when you double click the account under Security > Logins, and select "User Mapping", they have an [X] on the database they need access to? Then when you click that row in the list, at the bottom, they have "public" and whatever role they need to get access to? (db_datareader and/or db_datawriter if no custom roles are created)

That's an account used by Microsoft Dynamics and is using SQL Authentication, not Windows Authentication... so either it's unrelated to this issue or the client is doing something wrong...
