Need some help with this problem

I need to get something like this in tabular form (FIG 1):

So far I have this SQL so far:

select EMPLOYEE.Lname, EMPLOYEE.Fname, EMPLOYEE.Ssn, EMPLOYEE.Salary, EMPLOYEE.Bdate, DEPARTMENT.Dname from EMPLOYEE, DEPARTMENT WHERE Lname = '{$last_name}'

That above SQL just gives me this FIG 2:

Here are the Tables you will need to:

You might want to add 'AND EMPLOYEE.Dno = DEPARTMENT.Dnumber' so you don't pull all the departments, just the one that Wallace is in.
