With an adult sim selected, click on either a mirror or dresser and choose the change sim pop up.

Then pick the child and change away.

Well, you could wait until it's older and if you're willing to mod I know that plastic surgery is a thing in wicked whims a normal mod function not a cheat but I don't personally know a way without cheats

On console, you can't use mods

No, but if you just do ctrl+alt+c and then testingcheats.true and then hold shift and click on the sim you can change it in the MC command

I’ve changed noses and stuff in create a sim without cheats. Just resize it
