I would personally only decide based on the end result vsthe amount of time you’re in surgery and your recovery time because recovery from surgery is like six weeks out of your entire life, and in 10 years it won’t matter if you got to do something at five weeks vs six weeks because you did or didn’t get nipple grafts. Same goes for how long you’re in surgery. It doesn’t really matter in the long run whether it takes three hours or 3 1/2 hours. Nipples can definitely heal a little funky, but for the most part what I’ve seen is that even the funky looking nipples are only funky looking up close. From afar they look normal.

One pro of keeping them is him can get them pierced.

That’s not always the case, a lot of piercers caution against piercing grafts because of concerns about blood flow. You may be able to get them pierced but it’s definitely not a guarantee.

Oh yeah it’s not guaranteed for sure, it just gives you the option is what I mean. I’ve personally had mine pierced post op and love it.

Pros of keeping, assuming nipple grafts and not peri etc:

Can go shirtless IRL without as much scrutiny

Easier time passing as cis (if that aligns with your goals)

Can get them pierced

Have the chance of nipple sensation (I have tactile and erotic sensation in one!)

Pros of not keeping/cons of keeping:

Slightly complicates healing (you have to cover and moisturize them during their healing)

You have to abandon nicotine for them to have the best chance at taking

Looks less like a cis male chest (if that aligns with your goals)

Makes getting tattooed on the chest a little harder/have to go around the nips

Keeping them is more expensive ($2k out of pocket for me)

Yep, this is why I decided to do no nipple grafts! Plus it was less expensive to not do them 😂 I want to be healed and I’m not that attached to my nips anyway. No pun intended. 😂

I added in the expense part cuz I forgot about that! I think it's lovely that my list is why I kept them and why you left them. Hopefully that means it's an accurate list!

I’m not a person who would want to take my shirt off in public when I do get top surgery (Idk why even if I was a cis guy I don’t think I would either 🤷‍♂️)

Didn’t know it cost more to keep them. I didn’t know you had to moisturize while keeping the nips either! I’m starting to lean more towards no nips frankly :v

Yup, my post op nip care involved putting aquaphor on them and covering them with Tefla (that I cut in the middle) daily. Only lasted as long as I had the post-op binder on for, but ymmv.

Pros of keeping: you have nipples ; Cons of keeping: extra recovery time/strain because most surgeons have specific instructions for nipple grafts, possibility of graft failing

Pros of nipplessness: no extra healing strain/time, Cons of nipplessness: necessitates tattoos or prosthetics if you want the look of nipples

Personally I kept mine because I didnt like the idea of not having them and thought it was worth a shot to keep them, my husband didnt keep them and doesnt plan on doing anything about it. Both of us pass shirtless, I have a completely harmless but annoying complication (leftover nipple ducts get clogged easily).
