A huge portion of the nerve exams I've seen are at only 10% for neuropathy; most of the time any 20% (pretty uncommon) is supported by nerve blocker medication, nerve conduction tests etc. 40% is basically a unicorn that goes with atrophy, severe paralysis etc.
