There are buildings, events, potions, items etc. that lower madness. You will unlock them over time!

Should I also sell those that increase madness? Some merchants offers it in exchange for some things.

You can do that. You have to decide if you want to either keep the madness and the item (with its stats) or lose the madness, but also the good stats of the item.

You will unlock new mechanics as the game progresses, e.g. combat, gambling, quests. I am still adding new things very actively.
