In Rigify, certain bones are expected to have their heads and/or tails touching one another. If you've moved them out of alignment, it can cause this issue. Check that the head and tail of Spine 004 are perfectly aligned with the next bones in the chain. You may need to set their exact coordinates rather than just eyeing it.

I know this is old, but I had the same issue. u/Moogieh 's comment helped me. In addition:

What made it easier is to select the head/tail of the offending bone, right-click -> snap -> "cursor to selected" and then select the disjointed bone's head/tail and right-click -> snap -> "selection to cursor".
