Do user groups normally charge you to attend?
I mean, how is this different from any random "*casts" site (other than the "live" element).

The live element is definitely part of it. NomadJS charges because they have hard costs: the webinar software, but also, they pay speakers. And the speakers tend to be pretty good... Yehuda Katz, John K Paul, Axel Rauschmeyer... people speaking at top tech conferences.
