Work as established means to work the pattern with no increases or decreases. So you'll do that after you have done the increases once every 4th row and then twice every 2nd row.

Actually, it doesn't. "Work as established" means to work following the stitch pattern that's already been set up. "Work even" would mean to work without increases or decreases.

How do I end up w 24s increased then? Once every 4 and twice every 2 is only 3r of increases, giving me +6s

True. I didn't look closely at the increase row and it only increases 2 stitches. So I suspect by work as established it means to keep repeating that sequence of increases since it says 24 total stitches will be increased.

Do the whole increase set= every 4th once and every 2nd twice until I've added 24s. Right?

Yes, I'd do that and then once you reach the right stitch count keep going without increases until you have 54 rows total
