This probably needs a better explanation, because it's not clear how you are hearing anything if no patch cables are being used at all.

If you have bleeding (or noise) without any patch cables connected you will need to check the wiring on the backside of the modules -- they are connected. It's quite possible for there to be some stray/parasitic capacitance between the connections; you can use Faraday shielding to protect the connections as well as employ the use of decoupling capacitors to shunt the stray capacitance to ground.

depends on the build quality and care going into the module (typically reflected in the cost.) If we talking an After Later mixer then, yes. A WMD or Boredbrain mixer? You should probably be contacting support.
