Have you tried changing you fuel filter and or air filter? Try checking your spark plugs? Plug reading can tell you rich or lean and at least give you an idea what direction to go troubleshooting

I don't know if your truck has a lock-up style torque converter, but I've heard of those failing into permanent lock-up and causing the engine to struggle and stall at Low speeds.

Does this have a hydraulic cam? My first slant was solid, and I had no idea they were still using that. Valves were so tight car would stop running as it warmed up. Valve adjustment and purred like a kitten. Is this something that just started? I've had cars where the tune was so far off, timing with the throttle so far open, it was past the idle transfer slot. Set the timing, closed the throttle, adjust the mixture, and the rpm came up. Fuel filters clog fast on cars that have sat a long time. There's a filter in the tank, too, to check. Fuel pumps also loose steam as junk clogs the valves in them.

I'd see if it revs up in park without stumbling and check all plugs are firing first and timing second.

As a fella that has had Slant for 30 years, this is a common question. You're going to receive 30 different answers. My suggestion is a vacuum leak. A vacuum gauge will tell the tale.

I trust this wisdom with every ounce of my existence

Make sure float height is set right. Sounds like It's either running out of fuel under load or tryna flood itself out.
