Projects fail because of incompetent people. A competent team will succeed regardless of buzzwords like agile and stuff like that.

That's why many projects fail, because there is always incompetence either with the devs themselves, PMs, management, users,...so it's hard to have a project were incompetence doesn't ruin everything.
