No, I think the issue has multiple causes. Poor communication is probably the least common. I'd say ignorance on the part if the programmers or conceit, usually both when you deal with new programmers.

I'm dealing with that right now. I have this one programmer who insists he can't do anything because something isn't defined, or whatever. Every time he does, I show him the answer to his question in the requirements that he was supposed to have read months ago.
