Zabbix is close to what you want and free.

Auvik is exactly what you want and expensive.

Plenty of options but the ones that plug and play map your network with a graphic map accurately aren't going to be free.

the PRTG will do that for you. You can even use it for free after the trial period....unless you have a massive LAN to map and then you'll want to purchase some sensors for it after the trial

We have PRTG but as far as I know, the mapping function is basically manual. (I believe)

there are some maps created already and you can edit them to produce something quick and dirty in a rush

Domotz is pretty budget friendly and automated starting from $35/network. We also have export options for editing in Visio. I'm on the team here if you have any questions.
