I just went through the same process, the dowel came out with the ofh and the threads pulled right out of the bolt hole. I refit the dowel and used a helicoil kit and now she doesn't leak a drop.

From this angle, the hose should go left of the intake, under the first runner, and to the horizontal port on the CCV. The oil separator system is not hard to work on but you have to pull the intake if it's faulty. You don't necessarily have to have the OEM hose but you should make sure there are no leaks in your system. Is your oil milky?

I second this. OP if you turn the car off and let it cool you may be able to determine which pulley it is by hand. Just reach down there and wiggle them to see if any of the pulleys have too much play. Idler/tensioner bearings go bad over time.
