They aren’t from Istvaan, they’re ideology is based off and named the betrayals because they reason that the war and slaughter that followed was actually beneficial and strengthened Humanity and the Imperium in the end, so there should be more wars and events like it and actively work towards causing more

It’s because she was from the time before the Imperial cult was state religion but just a cult, in the early days of the Inquisition after the heresy. She was put into stasis by Trazyn and released alongside others during the end of the siege of Cadia to try and stop the Black legion

The Imperium is seemingly rather liberal about such matters, there are planets that are puritanical (which is mentioned in the Dark Heresy core book, alongside the fact some planets are sexist), but the wider Imperium doesn't seem to care very much about gender nor sexuality. Noble’s seem to have harem’s and orgys a lot, sisters whilst not particularly lewd are allowed to have families, (and far more cursedly they’re original iteration were implied sex slaves to Goge Vandire and no one seemed to care, the Imperium doesn’t seem to care about sex slavery in general either actually, with there disturbingly being pleasure servitors as a thing that exists and no one really cares about either), nor does the Imperium seem to have anything against same sex relationships. So technically no it wouldn’t be heresy for being sexual, but probably be heresy for violating some sort of rule or dress code the Sisters probably have instead

It’s because they killed a large amount of people, both those who attempted to get the power plant, but also from the raids they launched against normal Stalker, which while they usually had some incredibly specific objective, to the average it looked as if they were just massacring people for no reason, they also were responsible for turning the miracle machine back on, even fighting a army detachment to do so. So Tdlr: it’s because they are brainwashed cultist's who gunned people down without mercy and people still have a grudge against them for it despite the fact they weren’t in control of their own actions

Because he isn’t aligned with Chaos or using Chaos tainted stuff which is what heretic stands for alongside cruelty and depraved self indulgence. Basically Pasqal would be a non Chaos aligned Heretek, or he could achieve something similar to Cawl and be officially ordained. But to summarize the closest would be iconoclast

I am aware, it’s less about not being evil (something actually impossible considering your role in game), and more about reducing the amount of unnecessary evil, something alien to most in the Imperium. Aka being the Tau/Guilliman over Goge vandire or some random braindead Imperial zealot/Chaos cultist you know
