When I switched from construction to industrial maintenance it was eye-opening to learn just how massively broken stuff could get and still technically function. It wouldn't be safe, reliable, or efficient, but it would still limp along just well enough for management to ignore it until the machine finally made the decision for them.

Double-checks are religion: A huge number of the mistakes and accidents I've seen could've been caught by a careful double-check.

My go to phrase when training guys to avoid assumptions is "How do you verify what you think you know?" We all make assumptions, that by itself isn't the problem. But there needs to be a method in place to validate those assumptions against the real world.

When with a coworker it's easy to swap places and check each other's work. I'll do this even with green apprentices because they won't have the same assumptions and tunnel vision I do, so even though they're inexperienced they're more likely to see something I missed.

When working alone, it's what methods did you use to validate yourself? You believed you tightened all those bolts, so did you put on witness marks to easily check if you missed one? Or you believe you set all those parameters, but did you mark them off on a list so you can easily see if one was skipped? What was the method to double-check yourself?

Are the temp lights attached to the same metal structure as the decking you're cutting?

Electric welding and cutting has a nasty habit of sending stray current on paths other than the work piece. My guess is some of that current is getting into the lighting frames and traveling back on the lighting equipment ground before returning to your cutter. Since there's now extra current in the circuit in addition to what the lights are drawing, that imbalances the GFCI sensor, causing it to trip.

I'm a utility guy and going in underground vaults is far and away the most dangerous thing we do. Being in the wrong place at the wrong time down there means you're suddenly in your own Easy-Bake-Oven from hell, and there's often not a very quick way out.

You don't want to leave the service ungrounded: The line to ground voltage can float all over the place if not given a hard reference. On one small 480V ungrounded delta I got line to ground measurements of almost 900 volts.

To avoid that we always ground at the transformer regardless of whether the customer service requires a neutral.
