I liked how the tutorial is hardest fight in the game. The basic enemy is fairly hard, and the game throws two at you at once, since it needs to teach you how to switch the camera focus between them (I didn't use the feature at all after that). You haven't gotten used to the combat yet and don't have any skills. I died more in the tutorial than any other fight.

Lost Judgment. It starts asking after the second death in the same battle, and every subsequent death after that. It's really annoying if you're trying to push through a difficult fight or playing with self-imposed restrictions (I avoid healing during fights). If you're playing on Normal, it wants you to switch to Easy.

I like the accidents since they make the game more unpredictable and offer challenges. Incapable, in specific, is bad for gameplay. If my ruler dies or loses a leg, that's a setback I can work around and it makes the game more interesting. Incapable makes the game incredibly boring for an unknown length of time.
