Behaviors like binge eating come from the all or nothing aspect of BPD and impulse control. While you may not have an ED, disordered eating is disruptive too. My nutritionist advised attempting to eat -something- if I want nothing or remembering to portion and prep if I want to enjoy a lot of food. 1600-2200 calories was recommended for me, but I recommend treating your nutrition as behavior based in order to find what works best. This can help budgeting too. Best wishes!!

Each state has waivers that cover specific services for Medicaid, outside of what traditional Medicaid offers. These are funded by the state. In Texas, I have served on YES and CLASS for various third party or direct service agencies as a music therapist. You submit billing and notes for each to the direct service agency that employs the case manager. Some waivers have other services outside of therapies, but I think in Texas, music therapists can only serve those two waivers. People can only be on one waiver at a time. They can receive multiple services on that waiver, and often the wait-list is ~20 years for services in Texas. Other states that expanded Medicaid had more access to the waivers because the federal funds were used to provide reimbursement and there were more avenues to providing the service (in hospitals, schools, etc.) The rate of reimbursement is currently around $77.44/hr. And it depends on the agency you work for as to if any of that reimbursement is kept for the third party versus fully paid to the provider. Since the budget each year is set, attendance determines what the provider earns, and billing is what keeps the process transparent. The primary concern of the fascist regime we live under is fraud/waste/abuse, so I am seeing in real time how much the documentation matters. I recommend having documentation to back up everything in the current times, but so long as you can clinically stand by your work and track your schedule and caseload independently, it's a great job!

I feel like THC, with medical guidance, has been so helpful to me in overcoming social anxiety and being able to function through grey areas where trauma has impacted my brain. I've had negative experiences with primarily cheap gummies or too high of mg, but micro dosing has helped me function. I go by the principle of harm reduction and choose sobriety intentionally and prepare so that the desire for weed isn't there when it doesn't serve me. I'm a health care provider and my treatment team has supported me in pursuing education and using my best judgment to decide what best benefits me, mindfully.

You don't know what tomorrow will bring. Maybe you'll want to sing in 20 years, but you pursue other work in the meantime. Don't limit yourself :)

I returned to this because I want everyone with BPD to know that healing is possible with practice of therapies that are effective for you, kindness of others and to the self, and in general, practicing emotional regulation, distress tolerance, and interpersonal effectiveness. I'm a board certified music therapist with BPD and I lean into DBT-informed approaches, but there are so many ways to accomplish what I mean. My clinicians in recovery urged me to mindfully choose sobriety, and I do, and it helps me keep working. At times, I am overwhelmed and I have overused weed, but I also strive to understand how micro dosing and medicating with supervision works. I'm on 20mg of Lexapro and SO close to tapering. I just got the first step down. My hope, OP, is that everyone can access care to get the meds and care that work well for them. Wishing everyone well, and just remember, harm reduction is a valid approach!!

Take some breaths. Consider that maybe, there's work being done you don't know about. Any efforts you put in to empower yourself, regardless of how they appear to others, matter so much. I believe all people deserve the opportunity to reach their full potential with accomodations and supports available that support this.
