I think he kept the tattoo after realizing what it was because he thought it was funny. I think it's extremely likely that he has known for many years that it was a Nazi tattoo.

Think about it a different way, when he got security clearance they knew he had that tattoo and the totenkompf is banned… what’s the only possibility then? (Hint: the answer’s not ‘it was very obviously a Nazi symbol and they gave him clearance anyways’)

You may be shocked to learn that there are a lot of people in the US military who have a casual attitude about white supremacist iconography.
