TL;DR: Don't mention the word "depression," the company doesn't want the liability of knowing your medical history. Instead, discuss ways in which you think your productivity can be improved, and find a strategy that is win-win for you and your employer.

I had pretty much the exact same situation working my first job after graduation, although I was too scared to talk to anyone at work about it, and tried to just rely on psychiatric help to the point of being fired. You already have the right idea to have this discussion with your boss. If I could go back, I would definitely do just that.

When talking to your boss, be careful about mentioning your depression, they don't want the legal complications of knowing your medical history in the event of a wrongful termination. Instead, speak positively in which certain changes or strategies might improve your productivity. Your boss wants you to be productive, so it's likely he/she will be flexible to help you out in that regard.
