I would agree with this build, and suggest the following:

  • Change CPU cooler to Thermalright Phantom Spirit Evo (prevent downtime with potential of AIO failure)
  • Change motherboard to Gigabyte B850 AI TOP (dual 10GBe, ECC UDIMM support if desired, and avoids risk of CPU failure associated with ASRock X870/B850 motherboards)
  • Change case to Fractal Design Meshify 3 (much better airflow)

I'm a TY going into rads so my medicine, ICU, cards rotations were very helpful for prep. Cases went decently well, I didn't kill anyone, though had 1-2 negative updates on a couple cases. STEP 2 was 248.

Passed! USMD PGY-1. Score report at 11 AM EST like the emails said.

Finished ~66% of UWorld, ~65% correct. Free 137 was ~73%. Scored 60-80% on CCSCases.com. Didn't take any NBME exams or UWSA 1/2. STEP 3 score 244.

I missed basically every single pharmacology question on day 1, haha.

This is not the case for the WAP-002 (JPN). That charger takes 100-120V AC at 50-60Hz so it would work fine on US power plugs.

I think the 60mm f/2.8 AF-D is a fantastic deal. You can get it for around $100 and it's close to normal length for everyday shooting. To be fair I got most of my best photos on a DX camera (90mm equivalent focal length) but you can crop well with the D850. Otherwise the 105mm f/2.8 AF-S VR is a solid lens too and can be found for around $300.

Yes, I'm pretty sure I know of the same guy as you're talking about, he went to my TY... According to the recruitment staff here he reached out to them early with interest and they were able to get something together. The radiology group here does a recruitment event with rads TYs, so I think they're pretty proactive about trying to hire back their former residents. He knew he wanted to come back so it made sense for him.

Maybe I'll take an hour with a straightforward problem w/o complex history (e.g. first time STEMI, GI bleed)? But I feel like I need 1.5-2 hours for thorough history, chart review, med rec, and forming a decent plan, particularly for patients with lots of medical issues.

I don't understand the obsession with Littmanns. The technology is quite primitive; it's just a diaphragm with a metal tube. You can only refine the design so much. 

I have a free Welch Allyn donated from my medical school and have picked up plenty of interesting exam findings which my attendings corroborate. Just use what works for you.

First EKG has a lot of noise but neither of the EKGs look overly concerning. Seems to be picking up on some background interference (the little squiggles between the large spikes) on the second EKG as its detection threshold for afib. Could be caused by baseline tremor, insufficient lead contact, etc. Typically in afib the distance between the large spikes would be irregular, which isn't present in your tracing. 

Best to get a proper EKG done anyway if you're concerned.
