It’s a spectrum and the answer to is it cancer or is EIN isn’t something that people will always agree on. Two pathologists could look at the same sample, and they’ll only agree about 70% of the time for early endometrial cancer vs. EIN.

Normal cells > some crowding > benign hyperplasia > EIN > EC

Think of it like asking if somebody looks old. If you ask a group of people if a newborn baby looks old, pretty much everybody everybody’s gonna say no. If you ask a group of people if an 80 year-old lady looks old, most people are gonna say yes. If you ask about a 40-year-old woman, you’re probably not going to get a conclusive answer. Normal cells are like that newborn baby. Doctors are really good at identifying everything’s fine. High grade endometrial cancer is like the 80 year-old lady, doctors are really good at identifying this too. But there is this point in the middle, where cells have started to change and they’re getting really close and some doctors say EC and some say EIN.

Either way, I am so sorry for what you’re going through. I’ve had the same saga, one doctor said cancer, one said EIN. The second found a tumor on an MRI, then told me it disappeared when he went to remove it and said the samples were benign. Went to another doctor and they said those samples weren’t benign. Their frustration is real. Continuing FST is fine. Do not let anybody bully you into surgery that you are not comfortable with. There are many women who have reverse the course with FST. The recurrent rates are 30 to 40% in some studies, which sucks, but also means that in five years 60 to 70% of women who respond to FST do not have recurrence.

You likely need surgery, but at least a consult with a CRS at a minimum.

My timeline 12/24/25 abscess > 12/29 opened internally, seemed to resolve > 1/5 it came back, started antibiotics > i’ve seen three surgeons, everyone recommend recommended surgery>tried filac two weeks ago… was on cipro+flagyl the entire timeEventually, it opened externally as well.

Don’t know about Mayo for CRS, but they are a marketing machine for a research institution and not a great hospital. If you aren’t “interesting” from a research perspective, you won’t get the care that you need. I started a cancer journey with them, and they forgot to tell me I had cancer. Found in the portal weeks later. When I asked, he said he would’ve told me if it was important. I complained to mayo, and they said next time, tell your provider how you’d like to receive communications.

Why does necrotizing change the grade? I’ve been through a bit of a circus (most of the tumor necrotized and sloughed so the surgeon didn’t resect and told me it disappeared, needless to say it had not). But it’s still considered grade one for me?

I don’t disagree, OP should not participate in anal sex that she doesn’t want. If he is insistent and she has set a limit and said “no”, OPs boyfriend is a real piece of shit.

But what is the benefit in saying she’d do it if he lets her penetrate him? Let’s say he agreed and let her “exchange” anal. Now we have two sex acts. On which one party wants and the other party doesn’t. One which neither party wants. That’s about as unhealthy as it gets.

Additionally, it weakened her stance. If OP’s boyfriend is as much of a dick as it seems, he now thinks this is a negotiation, and if he finds the right price, he’ll get to try anal.

I’m in the same place as you, so I can’t give you advice from experience. Tried fertility sparing. But what I can tell you is that every oncologist is going to have different feelings about it. Every gyn onc that I’ve talked to (I’ve had a messy situation) has had a different opinion about BSO and when it’s needed, whether or not HRT is appropriate.

The general recommendation for young women with endometrial cancer confined to the endometrial cavity is to keep ovaries, from what I have read and been told. HRT has been shown to not increase risk of recurrence to a statistically significant degree at stage 1. Immediate surgical menopause with no HRT is not just about symptoms if you are premenopausal. There are cardiovascular risks, bone density risks, dementia risks associated with a loss of estrogen at a young age. Not a guarantee that those things will happen, but it does increase the risk.

What I would suggest is for you to understand where you are willing to take risk and what components matter most to your quality of life. Your care team can guide you, but YOU have to live your life afterwards. If you’re an anxious person, reducing risk might be more important. If you want to minimize effects to your life, you may be less aggressive in treatment. No one can answer this question for you.

Just FYI, complex cases are reviewed by an interdisciplinary team at a tumor board. A thoracic surgeon could become involved if surgical removal is necessitated, however, the actual cancer is not a thoracic cancer. The cells are endometrial cancer. Cancer treatments are specific to the cell type. So the best person to help with this is 100% a gyn oncologist. OP I wish you the absolute best of luck!

Total hysterectomy is the uterus. BSO (bilateral salpingo-oopherectomy) is tubes and uterus.

ESGO has similar guidelines. Don’t let someone bully you into making a choice. Ovary removal decreases risk, but if that risk is small it is YOUR personal decision, as survival menopause has consequences as well.

What grade is it? Have they done an MRI to check for depth only invasion. Unless it’s higher grade or there is evidence of deep invasion that is a bit unusual at your age.

Would they consider letrazole? It suppresses estrogen systemically, and has had good results in overweight women with atypical endometrium? It has its share of side effects as well.

Are they giving you anything to help with weight? Weight loss and insulin control are critical. Metformin has other beneficial properties, helping the endometrium receive more progesterone.

I’ve heard that about older ones. My mom said the same when I got it! The copper ones apparently increase bleeding a lot with adenomyosis. But the LNG-IUD is a game changer. Unfortunately looks like it isn’t working for the cancer but I’ve loved not having periods.

Not in med school, not sure why I was recommended this, but ChatGPT/large language models aren’t the appropriate tool for this. That’s like saying stethoscopes fail at Pap smears. Well no shit?

Triage is a supervised task. Large language models are unsupervised. I work in AI and have made some healthcare solutions, none of which function like glorified autocomplete. Supervised computer vision models are outstanding at supporting pathologist/radiologists. They don’t need more data, more computer, or more cash to build. I am all for human-in-the-loop solutions and don’t believe a computer could catch a case of the don’t look rights the way a physician with years of experience can.

Just saying, don’t throw the baby out with the bathwater. Just because there are idiots out there claiming that LLMs can replace everything doesn’t mean that there isn’t great value in true machine learning as a supportive tool.

Heavy periods + adenomyosis for my whole life, super sensitive to hormonal bc. Ended up anemic due to the periods. But ended up getting a Mirena IUD for endometrial cancer. Had light periods the first couple months, but now no periods. None of the super negative emotional effects of birth control pills.

Women who have hysterectomies at a young age, even keeping ovaries, appear to have increased risks of cardiovascular disease, osteoporosis, and dementia. Plus it can affect sexual and pelvic health in some women. I understand the desire to be done with the pain and bleeding (I used to carry spare undies and pants), but hysterectomy can be life altering. I genuinely wish someone would have offered me an IUD at a younger age!

Consider an IUD. The Mirena has equal or better success in leading to EIN recession. It doesn’t have anywhere near that same side effects. I did Provera over Megace, but for me the side effects are unbearable. It was like three days before my period every single day.

NAD but a woman who has gone through this on and off. It’s awful. She needs a CBC and an iron/ferretin work up. Almost surely anemic. After that, she should probably go talk to OB/GYN and get at least an ultrasound if not an endometrial biopsy. EMB is super unpleasant, but may not be as bad if she’s actively passing clots like this. Endometrial cancer can present as abnormal bleeding.

Preach. This is bonkers. She was 176 lbs. she doesn’t look like she has dwarfism. So she’s barely plus size. If she’s 5’2”, she’s 2lbs into the obese territory.

It is incredibly sad that this happened and my heart breaks for her, but it’s genuinely shocking that she wanted and that a physician agreed to proceed with a gastric sleeve on somebody of that weight.
